Playcentre Education Enrolment Pack

To enrol in the adult education programme

Download the following enrolment forms from the bottom of the page, complete and return to [email protected] with your name and the type of delivery in the subject field. I.e. Smith Chloe Webinar, Pere Aroha iQualify or Jones Robert Face-to-face.

  • Police vet form – complete the two pages provided below.
  • Safety check form
  • Recognition of Prior Learning (RPL) – only if applying for credit from previous study

Please note:

• We need the forms to signed with your signature, not just a typed name. There is a good (free) app called Adobe Fill and Sign which allows you to type in the required fields, and then still sign your signature online.  Otherwise, you will need to print, complete and scan clear documents, and return by email to [email protected]. Alternatively, you can post them back to Playcentre Education, CNI Regional Playcentre Aotearoa Office, PO Box 5252 Hamilton 3242.
• Copies of ID need to be verified by a Playcentre Aotearoa staff member, such as a Session Facilitator, Space Facilitator, Centre Advisor or Workshop Tutor.  Alternatively, you can contact the Education Team to set up a video call to verify your ID. Copies of driver’s licences must include the front and back since the expiration date is now printed on the reverse side.

Please send back completed and signed copies of:

• Enrolment form
• Safety check form
• Police vet form
• Recognition of Prior Learning (RPL) – only if applying for credit from relevant previous study
• Verified copies of primary AND secondary ID – see page 2 of How to verify identity documents for police vets for a list of ID options
